My Wellness Worksheet

Use this worksheet to define and prioritize your own goals for wellness. Writing down your goals and posting them in a convenient place (think refrigerator door) commits you to the process and reminds you of these goals daily. You may chose only one wellness goal or perhaps you wish to improve on one item in each wellness area:
- Emotional health
- Physical health
- Exercise
- Nutrition
Once you write down your goals, the next step is to think about the steps that will get you there. Think small steps first. For instance, if you are overweight and wish to improve your diet and exercise more you might begin with small action steps such as:
1. Add 2 servings of fruit and vegetables to my daily diet.
2. Increase the number of steps I walk daily (as measured by my pedometer) by 20%.
Once you have reached success with these steps you have the confidence and motivation to continue on the road to wellness with more successful lifestyle changes.
